Whether it is surveys, feedback, polls, measuring satisfaction or placing orders, using a form your users can send you any kind of information from anywhere, anytime.<\/p>\n<p>With\u00a0<a href=\"https:\/\/www.process.st\">Process Street<\/a>&#8216;s Forms you can enhance your team&#8217;s ability to collect data. Your Form responses can trigger <a href=\"https:\/\/www.process.st\/help\/docs\/what-is-a-workflow\/\">Workflows<\/a> or send that data to other apps.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-16726\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p><em><strong><i>Users: To create, edit, or share Forms, you must be an Administrator or a <a href=\"https:\/\/www.process.st\/help\/docs\/what-is-a-full-member\/\">Builder<\/a> with &#8220;edit&#8221; permissions.<\/i><\/strong><\/em><\/p>\n<p>You can enable <a href=\"https:\/\/www.process.st\/help\/docs\/version-control\/\" target=\"_blank\" rel=\"noopener\">Version control<\/a> to track changes to Forms for auditing purposes.<\/p>\n<h2>Common use cases<\/h2>\n<ul>\n<li data-stringify-indent=\"0\" data-stringify-border=\"0\">Intake Forms (e.g new customer or client)<\/li>\n<li data-stringify-indent=\"0\" data-stringify-border=\"0\">Contact Form<\/li>\n<li data-stringify-indent=\"0\" data-stringify-border=\"0\">Event Registration<\/li>\n<li data-stringify-indent=\"0\" data-stringify-border=\"0\">Surveys<\/li>\n<li data-stringify-indent=\"0\" data-stringify-border=\"0\">Feedback<\/li>\n<li data-stringify-indent=\"0\" data-stringify-border=\"0\">Applications<\/li>\n<\/ul>\n<h2>How to get started with Forms<\/h2>\n<p>If you have already built a Workflow, you may be familiar with the form fields that you can add to your form.<\/p>\n<p>To create a new form, click the new &#8220;<strong>+ New<\/strong>&#8221; button in the top-right corner and then select &#8220;<strong>Form<\/strong>&#8221; from the dropdown, as shown below.<\/p>\n<p>If you click <strong>New<\/strong> when in your <a href=\"https:\/\/www.process.st\/help\/docs\/library\/\">organization library<\/a> your form will be created there. If you&#8217;re in your reports dashboard, My Work, or any other part of the site when you add a new form, it will be created in your <a href=\"https:\/\/www.process.st\/help\/docs\/private-library\/\">private library<\/a>.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-8181\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-create-new.png\" alt=\"\" width=\"100%\" srcset=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-create-new.png 1024w, https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-create-new-300x150.png 300w, https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-create-new-768x384.png 768w\" sizes=\"(max-width: 1024px) 100vw, 1024px\" \/><\/p>\n<p><em><strong>Note<\/strong>: Only admins can create forms in the home folder in your Library. Builders and Users can create forms in <a href=\"https:\/\/www.process.st\/help\/docs\/folders\/\">folders<\/a> to which they have edit access.<\/em><\/p>\n<p>If you&#8217;re not sure where to start, you&#8217;ll find some great tips to help you learn the basics below.<\/p>\n<h2>Building a form<\/h2>\n<p>As soon as you start a new form, you will be prompted to name your form. You can also edit your form name after creating it, as shown below.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-8182\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-new.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p>The main body of your form is in the center of your screen where a short text form field is added by default. You can edit this field and add more <a href=\"https:\/\/www.process.st\/help\/docs\/form-fields-forms\/\">form fields<\/a> from the right-hand menu.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17165\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-form-fields.png\" alt=\"\" width=\"100%\" \/><\/p>\n<p>Next, you can add some content fields like text, images, or files to add some context to your questions.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17166\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-content.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p>Your forms can contain different types of fields to collect all the dynamic data you need. You can also add form field validations like default answers and placeholder text.<\/p>\n<h2>Auto-save<\/h2>\n<p>Your form is auto-saved every few seconds and will display the latest version if you refresh the page or return to the form later.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17170\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-autosave.png\" alt=\"\" width=\"100%\" \/><\/p>\n<p><strong>Note:<\/strong> You will still have to publish your form to update it to the latest version.<\/p>\n<h2>Form steps<\/h2>\n<p>You can add multiple steps in your form to segregate your questions. To do this, scroll to the bottom of your form and click the <strong>+ icon<\/strong> to add a new step.<\/p>\n<p>You can add form fields in the new step just as you did in your first step.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17173\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-add-step.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p>You can then <strong>rename<\/strong> your step, <strong>duplicate<\/strong> it, <strong>move<\/strong> its position in the form, or <strong>delete<\/strong> it. To do this, hover over the step name at the top of your screen and click the <strong>settings cog<\/strong> next to it, as shown below.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17174\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-steps.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p><strong>Pro tip:<\/strong> You can use these multiple steps to create &#8220;<em>if-this-then-that<\/em>&#8221; logic in your form. More on this <a href=\"https:\/\/www.process.st\/help\/docs\/forms\/#add-conditional-logic\">below<\/a>.<\/p>\n<p>These steps will also display a progress bar to the user while filling out a form response.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17219\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-step-responses.png\" alt=\"\" width=\"100%\" \/><\/p>\n<h2>Edit your form<\/h2>\n<p>Navigate to your form in your <a href=\"https:\/\/www.process.st\/help\/docs\/library\/\">Library<\/a>, click the <b>three dots<\/b> next to its name, and select <strong>&#8220;Edit&#8221;<\/strong>. You can also click your form&#8217;s name in the Library, as shown below.<\/p>\n<p>Now edit any of the form fields or steps, delete them, or add more.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-8183\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-edit.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p>You can edit your forms as many times as you need to keep them fresh and up to date.<\/p>\n<p>Make your edits, and click &#8220;<strong>Publish<\/strong>&#8221; to save your changes.<\/p>\n<h2>Customize your form<\/h2>\n<p>You can customize your forms with your <strong>brand logo<\/strong> and <strong>colors<\/strong> and by adding a <strong>background image<\/strong>, as shown below.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17168\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-custom.png\" alt=\"\" width=\"100%\" \/><\/p>\n<p>Learn more about <a href=\"https:\/\/www.process.st\/help\/docs\/customizing-your-forms\/\">customizing your forms<\/a>.<\/p>\n<h2>Add conditional logic<\/h2>\n<p>By adding conditional logic to your forms, you can further customize it to collect dynamic information in a single form.<\/p>\n<p>Navigate to the <strong>Logic<\/strong> tab to add <a href=\"https:\/\/www.process.st\/help\/docs\/conditional-logic-forms\/\">Conditional Logic<\/a> to your form.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17175\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-logic.png\" alt=\"\" width=\"100%\" \/><\/p>\n<h2>Publish your Form<\/h2>\n<p>When you&#8217;re done creating (or editing) your Form click &#8220;<strong>Publish<\/strong>&#8221; in the top-right corner to save your changes.<\/p>\n<p>As soon as you publish your form, you can choose to <a href=\"https:\/\/www.process.st\/help\/docs\/sharing-your-forms\/\">share<\/a> or <a href=\"https:\/\/www.process.st\/help\/docs\/embedding-your-forms\/\">embed<\/a> it, send it via email, or add automations.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17176\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-publish.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p>When you publish changes, any of your active forms also get updated automatically.<\/p>\n<p><em><strong>Pro-tip:<\/strong> Once you&#8217;ve published a form, you can start a new form response directly from your Library by clicking the <strong>Respond<\/strong> button next to the form name, as shown below.<\/em><\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-8184\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-respond.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<h2><strong>Form activity<\/strong><\/h2>\n<p>You can view all the recent updates made to your form in the activity log.<\/p>\n<p>Click on the <strong>Settings Cog<\/strong> in the top right corner then click <strong>Activity<\/strong> in the right panel, as shown below.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17182\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-activity.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p>You can also <strong>restore<\/strong> any deleted form responses from your activity log, as shown above.<\/p>\n<h2><strong>Automate your form<\/strong><\/h2>\n<p>To make your form super powerful, you can automate the actions to be triggered once a form response is submitted. For example, a new form response could trigger a new workflow run or send a Slack message.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17177\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-automate.png\" alt=\"\" width=\"100%\" \/><\/p>\n<p><strong>Note:<\/strong> If you create or edit automations, you need to publish your form to see the updates.<\/p>\n<p>Learn more about <a href=\"https:\/\/www.process.st\/help\/docs\/forms-automations\/\">form automations<\/a>.<\/p>\n<h2><strong>Responses<\/strong><\/h2>\n<p>You can view your form responses alongside your form editor in the <strong>Responses<\/strong> tab.<\/p>\n<p>You can filter your responses to see forms that were completed or are in progress. The response count shows you the number of completed forms. You can also filter your responses, as shown below.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17216\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-responses.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p><em><strong>Note:<\/strong> Users can view only their own form responses.<\/em><\/p>\n<p>You can also access your form responses directly from your Library by clicking the <strong>Responses<\/strong> icon next to your form name, as shown below.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-8185\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-responses.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p>If you want to delete any responses that were not filled out properly, select them using the checkbox to the left and click <strong>Delete<\/strong> from the floating menu at the bottom of the screen.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17217\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-delete-responses.gif\" alt=\"\" width=\"100%\" \/><\/p>\n<p>You can also export your form responses to CSV by clicking the export button in the top right corner of your screen, as shown below.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-17218\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/02\/forms-export-responses1.png\" alt=\"\" width=\"100%\" \/><\/p>\n<h2>Move, duplicate, archive, or delete a Form<\/h2>\n<p>When viewing your published form in the Library, you can move or copy it to a <a href=\"https:\/\/www.process.st\/help\/docs\/folders\/\">folder<\/a>, archive, or delete it.<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-full wp-image-8186\" src=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-moveedit.png\" alt=\"\" width=\"100%\" srcset=\"https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-moveedit.png 1024w, https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-moveedit-300x150.png 300w, https:\/\/www.process.st\/help\/wp-content\/uploads\/2024\/01\/form-moveedit-768x384.png 768w\" sizes=\"(max-width: 1024px) 100vw, 1024px\" \/><\/p>\n<p>Learn more about <a href=\"https:\/\/www.process.st\/help\/docs\/managing-your-forms\/\">managing your forms<\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Forms are the fastest and easiest way to collect information from a mass audience.
