Tracheostomy Care Steps

Gather all necessary equipment for tracheostomy care

Ensure that the patient is comfortable and explain the steps of the procedure

Don appropriate personal protective equipment (PPE)
4
Assess the patient's respiratory status and tracheostomy site

Cleanse hands and put on clean gloves
6
Prepare a clean area with all the equipment needed
7
Remove and dispose used dressings and secretions, ensure clean gloves are used
8
Inspect the stoma site for signs of infection or irritation
9
Clean the tracheostomy site from the stoma outward, using mild soap and water
10
Decision task: Decide whether suctioning is needed

If suction is needed, use aseptic technique and proper suction depth
12
Clean the inner cannula with hydrogen peroxide if it is not disposable
13
Rinse the inner cannula with sterile saline if it is not disposable
14
Reinsert cleaned or new inner cannula and secure it
15
Apply new sterile dressing around the tracheostomy tube
16
Remove gloves, clean hands and apply new gloves
17
Reposition the patient comfortably and provide oral care if needed
18
Document the procedure including the patient’s response and condition of stoma site
19
Dispose of used equipment and cleanse work area
20
