Cleanse the peristomal skin area with mild soap and water
Pat dry the cleansed area with a clean towel
Perform a thorough skin inspection for any signs of irritation or infection

Prepare the new ostomy pouch and skin barrier by trimming it according to the size of the stoma
Apply skin protectant to the peristomal skin area
Attach the ostomy pouch to the skin barrier securely
Ensure the ostomy bag is not more than one-third full

Monitor for any potential leakage from the ostomy pouch
Record the procedure including the condition of the skin, the size of the stoma, and reaction to the procedure
Dispose of all used materials and cleanse the area where the procedure was done

Explain the procedure to the patient and answer any questions they may have
