InJOY doesn't send everyone that has an accident to the clinic. We need to make the judgement call with the injured/ill employee as to whether they may require medical attention or if the situation on requires first aid.
Arrange Transportation
Typically we call Healthpointe to setup transportation for the employee. They have a contract with a cab company to provide transportation for their clients. Call Healthpointe at
There have been times when InJOY has taken employees to the clinic or asked the employee to drive themselves. This will be at the discretion of the Program Director and is a rare occurrence.
Clinic Authorization
Get Clinic Authorization form from the Worker's Compensation Binder and fill out the following information.

- Services Requested
-Typically W/C Injury
- Date of Injury
- Body Part
- Print
- Title
- Signature
DWC-1
Get DWC-1 form from the Worker's Compensation Binder.
Complete the employee section WITH the employee to ensure they are putting relevant staff information, not member information.
Complete the employer section.
Copy the form and send the copy to the clinic with the employee.
Accident Investigation
Complete the Accident Investigation Form with the employee. Verbally process through the accident with them to make sure accurate, staff related information goes on the form.
DWC-1 Claim Log
Fill out the DWC-1 Claim Log as the items are completed. Be sure to do this as the items are completed so the information is accurate.
If the employees recover takes longer than 1 month, it is a good idea to use a calendar to track the days the employee is on modified duty or off work due to the injury. We need to retain this information.
OSHA Form 300 Appendix A Log
Add the injury/illness to the OSHA Form 300 Appendix A. This log is required to be kept up to date within one month of an injury/illness. You may not have all the information to complete it, but log the information that you have.
