Give a short description of the event or problem so that we can quickly see an overview of issues and build a database of problems and solutions.

Whoever takes ownership of the event / issue will determine the level of response required.

If a simple solution is appropriate then it will be assigned with an action item.

If a more in depth analysis is required then this will be assigned to the appropriate team member.

If the incident can be fixed quickly and easily and there is no need for an in depth analysis, provide the details of the fix here and complete the checklist.

If more analysis is required skip this step by clicking next and go to root cause analysis.

Define the problem in more detail including specifics, location and the impact.

Think about the problem and ask why it is occurring. Search for an answer that is grounded in fact. Write your response to the first why.

Then ask why again for as many times as is useful. Frame the question each time in response to the previous answer.

Try to move on quickly from one question to the next, so that you have the full picture before you jump to any conclusions.
