Send staff to Concentra clinic IMMEDIATELY:
- To be evaluated and possibly treated
- To have a post-accident drug and alcohol test
They don’t make appointments- walk ins only!
Employee should take to the clinic the below:
*NO Chain of Custody form needed*
- Concentra - VitalMD Subsidiaries - Employer Authorization for Initial Treatment form
- Picture ID
- And Pharmacy card
At Concentra, the employee should state that they are there for:
- a 10-panel Post Accident drug screen (urine) and a Breathalyzer Alcohol test for post-accident
- initial medical treatment under Workers Comp
How to complete forms:
Concentra will provide the employee with a work status report. It is the employee’s responsibility to provide a copy of the work status report from Concentra to the Office Manager.

Monday – Friday 8:00 am - 6:00 pm
Saturday 10:00 am - 3:00 pm
If Concentra clinic is closed - send staff member to ER.
- At ER they can use their health insurance (save receipts to submit to Worker's Comp & Femwell HR ) for reimbursement.
- Will still need to go to Concentra (after ER) within 24 hours for drug and alcohol testing.
