Introduction to the Employee Evaluation Form:

Employee evaluation forms are used to formally review and document an employee’s performance.

Evaluating and providing feedback on an employees’ performance is often seen as an unimportant, time consuming task for line managers and HR representatives. In fact, research shows that 58% of companies say it’s not an effective use of time. Only 8% of companies believe their performance management process drives business value, and one in every five employees won't receive feedback on how they’re performing. 

However, as dull and dreary as it may seem, evaluating an employee and giving them constructive feedback on their performance is imperative for a happy, productive and effective workforce.

Not only do employee evaluations boost employee morale, confidence, and therefore performance, they also provide critical information for companies to use when making important decisions, such as promotions, pay raises, and layoffs.

Set the date of the employee's evaluation and enter the length of their evaluation period in months. 

Performance and Goals:

Rate the employees performance

Give a rating from between 1 and 5 to each of the disciplines below. Add comments to support the rating you have given.  

Add the ratings up and enter the employee's total score. Use this to work out their average score

1 = Poor
2 = Fair
3 = Satisfactory
4 = Good
5 = Excellent

Analyze the employees performance

Describe the employee's areas of strength and areas of weakness. 

Establish SMART goals

Based on the employees strengths and weaknesses, set five S.M.A.R.T goals that the employee can work towards to improve their performance. 

To make sure the goals you set the employee are clear and achievable, each one should be S.M.A.R.T:

Specific (simple, sensible, significant).

Measurable (meaningful, motivating).

Achievable (agreed, attainable).

Relevant (reasonable, realistic and resourced, results-based).

Time-bound (time-based, time-limited, time/cost limited, timely, time-sensitive).

Review the employee's strengths, weaknesses, and their five S.M.A.R.T goals.

Once you and the employee are satisfied with this information, you will both need to approve this task.
